When an employer is requesting medical certification for FMLA, is the employee/patient automatically waiving their privacy rights by accepting the Certification for Health Care Provider and giving it to the doctor?

Also, is "surgical procedure" enough or does it need to state" surgical procedure due to hernia"?

Each employer can differ, but you will need a doctor statement.
